This feedback survey is limited to partners who received a sample. Enter the work email your sample was issued to — survey questions appear after verification.
- The survey opens only after we match your email to a sample record — a verified sample code, a claim you submitted, or a handoff logged by your ATC rep.
- Use the same work email your ATC representative recorded. Personal aliases usually will not match.
